IMMUNIZATION PRE-REGISTRATION


Pre-register for your flu shots or childrens immunizations, offered at our Smyrna office.
Starting October 12 we will initiate innoculations for seasonal flu, h1n1, and childrens immuniztions (excluding polio).

You can email your name, phone number and prospective date you are interested in. Shots avalible on Tue, Wed, and Thurs for appt.  Walk ins avalible on Tue and Wed.

**the cost may be a minimal $13.00 -$23.00  or free to medicaid and peachcare children.
Note: please bring your immunization card with you to your first visit for children.
